Cannibal Holocaust is so violent and realistic, for many years there was a rumor it was a snuff film that depicted its actors getting killed onscreen. This Italian faux-documentary is still banned in a number of countries.
It was also a leading film in the ‘video nasty’ moral panic in 1980s Britain. Finally, its director faced obscenity charges in his native Italy.
This movie is truly horrifying, and if the cannibalism doesn’t turn your stomach, the animal cruelty will.

This 2010 entry has overtaken Cannibal Holocaust as the presumptive ‘most violent/ disturbing/ horrifying’ film of all time. Every person I know that has seen this film says it is worse than you could ever imagine.
Most horror films don’t live up to their horrifying reputations, but apparently the necrophilia, pedophilia, and sexual violence is worse than you could imagine. Someone rape’s a baby in it.

The Human Centipede (First Sequence) (2009)

It is the film where tourists are turned into a giant centipede by stitching their mouths to the others’ asses. Ah, the much-maligned rape-revenge genre. You knew some of these films had to make it onto a list of the most violent horror films, didn’t you?
