Saw V (2008)
Any of the Saw films could fill this spot on the list, but Saw V is especially sadistic. This film has loads of story and it doesn’t all work, but it also has the blood saw. The blood saw is a literal saw which collects blood from the trapped victim, which unlocks a door. Yeah, its sadistic and exactly the type of deranged the Saw movies did so well.
