Puff the Little Fire Dragon is the slowest roller coaster in Kiddieland at Lagoon Park in Farmington, Utah. But in 1989, it still managed to kill a 6-year-old boy who slipped from the safety of his car. After he fell, he tried to get back on the track, but that’s when the train hit him in the head and killed him.
