Hydro Death
In April 2004, a 16-year-old girl was on the Hydro ride, a roller coaster in the Oakwood Theme Park in Wales, when she fell 100 feet from the top of the ride and suffered fatal internal injuries. During the investigation, it was discovered that staff failed to inspect the seatbelts or bars.
